Scope of This Notice

This notice covers the public Greenberg Spine website. It is not a Notice of Privacy Practices for medical records and does not replace the privacy notice provided by the organization responsible for your clinical care.

Information the Website Collects

The appointment and second-opinion forms collect contact and routing information such as name, email address, phone number, request type, and the page where the request began. The free-guide email form collects an email address to deliver the requested guide and a backup link. The referral page does not collect or submit referral information: its email button opens a pre-addressed draft in the visitor’s email app, and the patient-information form is disabled. The prepared email requests only the patient’s name and best callback number plus the referring office’s contact information. The visitor’s email provider, not this website, sends the message directly to the Greenberg Spine practice mailbox. Do not include date of birth, symptoms, diagnoses, imaging, insurance information, claim details, records, attachments, or other clinical information in that lightweight referral email.

The site may also process ordinary technical information needed to deliver and protect a website, such as an IP address, browser and device information, requested pages, referrer, and basic conversion events. Campaign parameters in a link may be recorded with an aggregate event so the site can measure which outreach led to a call or form start.

How Website Information Is Used

Contact information is used to route a request, send a requested update, and allow the office to respond through an appropriate channel. Site-usage information is used to operate, secure, and improve the website and to measure aggregate engagement.

Patient Guide Delivery

The patient-guide form collects only an email address. We use that address to provide immediate access to the requested guide and send a backup delivery link. MailerLite processes the address for this delivery workflow on our behalf. Submitting the guide form does not enroll you in a newsletter or other marketing updates. The address is retained only for the guide-delivery workflow unless a longer period is required by law. You may request deletion through the contact form below.

Information Sharing

We do not sell or trade your personal information. We share limited information with service providers only when needed to operate the website, deliver a requested resource, communicate with you, or comply with law. Those providers may process information on our behalf for those limited purposes.

Service Providers and Disclosures

The website uses service providers for hosting, security, analytics, subscriber management, and delivery of office alerts. They may process information only as needed to provide those services. Information may also be disclosed when required by law or to protect the security and integrity of the site. Greenberg Spine does not sell information submitted through its website forms.

Clinical Information and HIPAA

Clinical records, billing information, and protected health information belong in the clinical systems designated by the responsible medical practice or facility—not in these public forms. The applicable Notice of Privacy Practices explains how that organization may use and disclose protected health information and how to exercise HIPAA rights. If care is provided through a Parkview Health facility or practice, Parkview publishes its current Notice of Privacy Practices. Confirm with the office which organization maintains the relevant record and which notice applies.

Choices and Requests

You may ask to correct contact information submitted through the website, unsubscribe from email updates, or ask a question about website data by contacting the office. Some information may be retained when needed for security, legal obligations, or an active request. Requests concerning a medical record must go to the organization that maintains that record.

Security

Reasonable administrative and technical safeguards are used for the public website, but no internet transmission is risk-free. Do not use a website form for an emergency or for sensitive medical information.
